Membership Application

• This application form includes a questionnaire about CineVic, so that new members can learn more about our organization before joining. All of the answers can be found on this website and in our Bylaws. We recommend opening the form in a new tab or window, so you can peruse our site to find the answers.

• You will need to provide contact information for 2 references who can speak to your filmmaking interest/abilities. If you know any current CineVic members, they make great references! Please obtain permission from your references before submitting their contact information.

• After submitting your application form, you will need to pay the membership fee at the CineVic Online Store or in-­person at our office. You will also need to come meet us in person during office hours (Tues-Fri, 9am-2pm) so that we can properly assess your application. Please note that your membership is not fully valid until you have submitted your form, paid your fees, met us in person, and the board has approved you.

• We encourage renewing members to also fill out this form when renewing your membership, to help us maintain up-to-date contact information. Renewing members are not required to complete the references or questionnaire sections.

• Please note that memberships are only valid April 1st to March 31st each year.
